3M Fall Protection (formerly Capital Safety) has identified a limited number of 3M™ DBI-SALA® Lad-Saf X2/X3 sleeves with an assembly error that may cause the locking lever to not work properly. This condition may allow the sleeve to disengage from the cable during use. This assembly error is believed to affect only a limited number of units and has been corrected. There have been no accidents or injuries related to this issue.
This Notice only applies to the X2 (#6160030) & X3 (#6160054) version of the sleeve. These sleeves are sold individually or as part of a system. Distributors who receive this Notice should contact our Customer Service department at 1- 800-387-7484 (prompt #5908) or email LADSAFNA@mmm.com to obtain a listing a parts and systems that were sold to you.
Please forward the attached Inspection Notice to any of your customers who have purchased the X2 or X3 Lad-Saf sleeves or systems from you. It is important for your customers to conduct immediate inspectoins as described in the Notice to identify affected units.
Alternatively, if you provide us with a list and contact information of your customers of customers who have purchased Lad-Saf X2 & X3 sleeves from you, 3M Fall Protection will communicate directly with your customers to facilitate the inspection and replacement of the affected sleeves.
